Any thoughts on why?. My own observation and understanding is that DL wanted to put a Sporting Director type person in place, continental-style to run the club. I guess like a 'Monchi' of sorts. Each time we've tried, it hasn't really worked.

People will criticise us going for someone like Frank, but I think our biggest mistakes in any recruitment is hiring someone who thinks they are better than us. I feel like Paratici is the first 'sporting director' type guy who genuinely wants to be here and loves the challenge.

I don't believe that we are hiring staff (with the odd exception) who believe that they are better than us,
We are just hiring the wrong people, time after time after time.
You would think that the club would have put a strategy in place to overcome the issues that have arisen now, but we lurch from one mess to another.
Munn and Ange are the latest in a line of wrong appointments. It was two years ago that we were waiting for Munn to be released by Man City and was heralded as the man that was going to sort everything out with Lange and keep Levy away from the football side of the club.
We are a mess and it is no surprise that there is boardroom infighting.

When we did sign players outside of the manager's myopic judgement, we'd have the likes of Conte refusing to play them (Spence, Danjuma?)

He’s signed some young talent, but has shown no signs, much like Paratici, of knowing how to put together a balanced side. You need silk, steel, youth, a bit of experience, goal scorers. Redknapp said it publicly when he wanted the England job. He also at one point mocked the fans about the club being rubbish.
